Validity
Up to 6 months (single or multiple entry) or 6 months to 1 year (multiple). Ethiopia is excluded from the default six-month triple-entry option, so the Mission decides duration case by case.

Single or multiple for the up-to-6-months tier; multiple for the 6-months-to-1-year tier.
Medical attendant visa
Up to two attendants, co-terminus with the patient's visa and at the same fee. Minor children accompanying a parent who is the patient may instead be granted an X-Misc visa. No blood-relation requirement is stated for Ethiopian nationals.
Vaccination certificates
A yellow fever certificate is mandatory — Ethiopia is on India's endemic list, and missions issue visas to applicants from endemic countries only after the certificate is produced. Without it you can be quarantined for up to 6 days. The certificate is valid for life, beginning 10 days after vaccination. The mission also asks for an oral polio card even though Ethiopia is not on India's polio-affected list, so carry it.

The hospital invitation letter
Required, and it must be generated by the Indian hospital through the official Medical & Ayush Visa Portal — a letter merely typed on hospital letterhead is not enough. The Ministry of Home Affairs format also applies: patient's name, diagnosis, hospital and place, consultant and speciality, cost and duration of treatment, passport numbers of patient and attendant, and an authorised signatory with contact details.

Medical visas are not free under the LDC concession
Ethiopia is on the Least Developed Countries list, but the gratis provision covers only Business and Employment visas. The India–Ethiopia visa waiver applies to diplomatic passport holders only.
