Nigerian nationals are not eligible for the e-Visa
The High Commission states plainly that there is no e-Visa facility for Nigerian nationals. Only 'Niger Republic' appears on the eligible list — not Nigeria. You must apply for the paper Medical visa.
The fee is US$588, not US$83
Nigeria pays a flat reciprocity rate for every Indian visa category. Budget roughly seven times the standard medical visa fee, in Naira cash, and note that the fee is the same for each attendant.
Which visa you can apply for
Nigeria is not on India's e-Visa eligible list. You must apply for the paper Medical (MED) visa at High Commission of India, Abuja.
Processing time
Not published as a day-count. The High Commission in Abuja states that processing times vary and are at its sole discretion; the Consulate in Lagos says some applications may take up to six weeks.

Medical attendant visa
Up to two attendants. The attendant pays the same flat US$585 + US$3, because the Nigerian fee table covers all categories at one rate. Proof of relationship between patient and attendant is required, though the qualifying relationship is not defined officially.
Vaccination certificates
Both yellow fever and polio certificates are mandatory. Nigeria is on India's yellow fever endemic list — without a valid certificate you can be quarantined for up to 6 days, and missions generally issue visas only after the certificate is produced. Nigeria is also listed by the Health Ministry among countries with poliovirus circulation, so an OPV or IPV certificate is required.

The hospital invitation letter
Required, typed on hospital letterhead, stamped and signed with contact details. It must state the nature of the treatment, the start and end dates, and the attendant's details with a contact person's mobile number, and must include an undertaking that the hospital takes responsibility for the patient and attendant until departure from India. The hospital must email it directly to the mission. Abuja additionally requires an FRRO-registered invitation carrying a file reference number — ask your hospital to confirm this.

You must apply in person — there is no VFS or BLS
All applications are submitted in person at the High Commission in Abuja or the Consulate in Lagos. Lagos covers the South West, South East and South South states; everywhere else goes to Abuja. Urgent medical cases without an appointment may attend between 9 and 11 am on any working day.
